We are looking to recruit an experienced and enthusiastic Continuing Health Care Advisor who will work across the Wakefield Health and Care system.
Purpose of the post:
To lead, advise and coordinate on behalf of the Local Authority in adhering to the duties and delivering the Local Authority’s responsibilities with regards to the National Framework for Continuing Healthcare and Funded Nursing care. This includes the Local Authority’s obligation to NHSE to participate in Independent Review Panels.
To work collaboratively with colleagues across the local authority and NHS Integrated Care Boards (locally and out area) to determine individuals needs and CHC eligibility against the NHS National framework for Continuing Heath Care.
To assess and determine health and social care funding splits for joint packages of care, attending and contributing to weekly joint funding panel with our ICB partners.
To represent the Local Authority under the local resolution and dispute procedures.
Provide training and day to day support and advice to Local Authority practitioners undertaking Continuing Health Care duties.
Contributing to the development of policies and procedures within the CHC service on behalf of the Local Authority.
