An exciting opportunity has arisen for a permanent full time (37 hours) Social Work position within Connecting Care East. We are an integrated team who work across the East of the Wakefield District with people aged 18 and over who have an appearance of need.
Connecting Care East is an extremely supportive and friendly team with an abundance of experience. As a qualified and registered social worker, you will play an important role in helping vulnerable people live as independently as possible, supporting and signposting to empower people to be autonomous about how their needs are met.
Part of this role will involve you taking responsibility for completing Care Act conversation assessments and developing person centred and outcome focused care and support plans. The successful applicant (if newly qualified) will be supported with progressing through a comprehensive ASYE programme to develop their practice.
Wakefield Adult Social Care has implemented a Social Work Career Framework to support the professional development of its social work practitioners. This enables Social Workers who have successfully completed the ASYE Programme to be considered for progression to the role of Experienced Social Worker.
If you’ve already completed the ASYE Programme with your current employer, this must be evidenced at recruitment. Once you have consolidated your learning 2 years post ASYE Programme completion and demonstrated to us that you’re working at or significantly towards the level of an Experienced Social Worker as defined by the PCF, you are able to nominate yourself for progression with your managers endorsement.
Wakefield Council offers a wide range of development and training opportunities for the successful applicant as well as a generous annual leave entitlement. We are adapting to hybrid working and we have flexible working in operation.
