Are you passionate about mental health? If so, an exciting opportunity has arisen for a full time Social Worker to join the Mental Health Social Work Hub. This bespoke adult social care team works closely with South West Yorkshire Partnership Trust Mental Health Services in offering a comprehensive, dedicated, social care resource for people with mental health needs. The team will provide assessments, support planning and commission services for people with mental health needs across the Wakefield District.
As a registered Social Worker, you will play a crucial role in empowering individuals with mental health needs to live independently and make informed decisions about their care. You will conduct assessments, support planning, and commission services tailored to each person’s unique needs. Experience in mental health is desirable, along with strong assessment and communication skills, and a solid understanding of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
As part of your role, you will undertake planned interventions, complete conversation-style assessments, strength-based reviews, and develop person-centred, and outcome-focussed care and support plans.
